AI models generate likely outputs from patterns rather than checking every statement against reality. A hallucination happens when the model produces a claim, citation, detail, or answer that sounds plausible but is not actually supported.

Example

An AI assistant might confidently cite a court case that does not exist or invent a product feature that was never documented.
